To commit to your team:

  1. Mail us your non-refundable deposit of $225 made payable to USP Select and put in the memo line "your teams age/sex"

  2. Mail us a copy of your Birth Certificate.

  3. Mail us 2 photos of your child.

  4. Mail us this US Club Registration/Medical Release form - #R002-Youth: Youth Player Registration & Medical Treatment Authorization Form
